Job Title: Registered Nurse (RN) Case Manager (Emergency Department)
Location: Hyannis Massachusetts
Work Setting: Acute Care Hospital (Emergency Department)
Job Type: Contract (13 Weeks Extension Possible)
Compensation
- Weekly Pay: $2800 $3150 (based on experience)
Schedule & Work Hours
- 40 hours per week
- 4 shifts per week (10.5-hour shifts)
- Shift options:
- 11:30 AM 10:00 PM
- Late shift rotation (including evenings/weekends)
- Rotating schedule including weekends
Job Summary
We are seeking an experienced RN Case Manager to support care coordination within the Emergency Department. This role focuses on discharge planning utilization review and ensuring patients receive the appropriate level of care while optimizing healthcare resources.
Key Responsibilities
- Assess patients for discharge planning and ongoing care needs
- Coordinate care with physicians nurses and interdisciplinary teams
- Perform utilization review using tools such as InterQual
- Ensure appropriate level of care (inpatient vs. observation status)
- Facilitate insurance authorization and reimbursement processes
- Coordinate patient discharges transfers and follow-up services
- Advocate for patients and communicate effectively with families
- Identify high-risk patients and connect them with appropriate resources
- Participate in care rounds and maintain accurate documentation
Required Qualifications (Non-Negotiable)
- Active Registered Nurse (RN) license Massachusetts
- Minimum 3 years of acute care hospital experience
- Experience in case management discharge planning or utilization review
- Knowledge of InterQual or similar utilization review systems
- Strong communication organizational and multitasking skills
Preferred Qualifications
- BSN (Bachelor of Science in Nursing)
- Case Management Certification (CCM or equivalent)
- Prior experience in Emergency Department case management
