Schedule & Shift Details:
- Evening Shift
- 02:30 PM 10:30 PM
- Every other weekend required
- Floating required within facility (same building)
- 37.5 hours per week (Guaranteed)
Job Overview:
Navitas Healthcare LLC is seeking a dedicated Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN)to provide quality nursing care in a long-term care setting. The ideal candidate will support patients with medical-surgical orthopedic dementia and memory care needs while ensuring high standards of patient care.
Key Responsibilities:
- Provide direct patient care in a long-term care setting
- Administer medications and treatments as prescribed
- Monitor patient conditions and report changes
- Assist with activities of daily living (ADLs)
- Document patient care accurately in EMR systems
- Collaborate with healthcare team members
- Support patient safety and infection control practices
- Float between units within the same facility as needed
Required Qualifications:
- Active LPN/LVN license (New York) required
- Minimum 1 year of experience required
- BLS certification required
- Experience in long-term care preferred
- Familiarity with EMR systems (PointClickCare preferred)
- Strong clinical and patient care skills
- Ability to work in a fast-paced LTC environment
