Schedule & Shift Details:
- Day Shift
- 06:30 AM 02:30 PM
- 5 7.5-hour shifts (37.5 hours/week guaranteed)
- Every other weekend required
- Floating required within facility (same building)
Work Setting:
- Long Term Care
- 40-bed unit
- Patient population: MS orthopedic dementia and memory care
Job Overview:
Navitas Healthcare LLC isseeking a reliable Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN)to provide quality patient care in a long-term care setting. The ideal candidate will support daily nursing operations and deliver compassionate care to a diverse patient population.
Key Responsibilities:
- Provide direct patient care in a long-term care environment
- Administer medications and treatments as prescribed
- Monitor patient conditions and report changes
- Assist with activities of daily living (ADLs)
- Maintain accurate documentation in EMR systems
- Collaborate with healthcare team members
- Float between units as required
- Ensure adherence to patient safety and care standards
Required Qualifications:
- Active New York LPN/LVN license required
- Minimum 1 year of experience required
- BLS certification required
- Experience in long-term care preferred
- Familiarity with EMR systems (PointClickCare preferred)
- Strong patient care and communication skills
