Job Summary
The Registered Nurse (RN) LTC provides comprehensive nursing care to residents in a long-term care and rehabilitation setting. This role focuses on medication administration patient monitoring and delivering compassionate care to patients with chronic conditions including Alzheimers and dementia.
Key Responsibilities
Administer medications and perform medication passes accurately
Provide direct patient care in a long-term care setting
Monitor patient conditions and report changes to the care team
Assist in developing and implementing care plans
Support patients with Alzheimers dementia and other chronic conditions
Maintain accurate documentation using systems such as PointClickCare
Ensure compliance with safety and infection control standards
Schedule & Work Details
Mid Shift: 5 x 8-hour shifts (2:30 PM 10:45 PM)
Every other weekend required

Required Qualifications
Active RN license (state-specific required at submission)
Minimum 1 year of nursing experience
BLS certification
COVID-19 vaccination required
Preferred Qualifications
Experience in LTC setting
Experience in rehab med-surg or hospital settings
Familiarity with PointClickCare
