Supported Assistant Practitioner/Assistant Practitioner 10338 permanent contract 37 hours per week 32061 to 33699 (G grade) or 29064 to 31022 (F grade) per annum GradeF/G Norwich

An exciting opportunity has arisen for us to recruit an Assistant Practitioner to join the Northern Mental Health Social Care Team covering the North Norfolk area as well as the Mental Health Duty Team.


We have embraced our new Community Social Work model Human Learning Systems looking at new and creative ways to prevent reduce and delay care and support needs. Early intervention creativity and utilising community assets are key as is improving our community-led support offer which will include engaging with people in public spaces. You will be joining a passionate forward thinking supportive and high performing team. You will undertake Care Act Assessments Support Planning Reviews and take part in covering our duty team whilst working collaboratively with partners including Norfolk and Suffolk Foundation Trust and various stakeholders to promote independence.


We are seeking an enthusiastic community Assistant Practitioner to join our highly motivated innovative team of Practice Consultants Social Workers Assistant Practitioners and Living Well Officers. It is an exciting time to join our team of dedicated and passionate workers in embracing our Living Well agenda and supporting people to be independent resilient and well.


The successful candidate would need to have knowledge and a level of experience of the Care Act (2014) Mental Capacity Act (2005) Mental Health Act (1983) and Safeguarding vulnerable adults as well as excellent time management and IT skills. Any new staff are supported to develop a thorough working knowledge of Norfolk County Council processes and procedures.


We operate a hybrid model of working which includes working from home and from our office base at Hellesdon Hospital. There are also opportunities to work from other Norfolk County Council bases. To build meaningful relationships with the people we support this role requires regular in-person work in both the office and community.
