What Youll Do

• Provide individualized care based on the clients Plan of Care

• Monitor and document changes in the clients condition

• Communicate proactively with the Director of Nursing and family

• Ensure safe high-quality in-home care

• Build a meaningful long-term client relationship

Requirements

• Active unrestrictedRN or LPN license in Indiana

• CPR certification & negative TB test or chest X-ray

• Previous nursing experience preferred

• Valid drivers license and auto insurance

• Ability to lift up to 50 lbs. and assist with mobility as needed
