Job Summary
POSITION SUMMARY:The Behavioral Health Care Manager is responsible for coordinating and supporting the behavioral health treatment of the patient by working directly with the patients primary care provider and psychiatric consultant. This position reports directly to the Director of Value Based : Licensed clinical social worker with behavioral health experience nurse practitioner with behavioral health experience or psychologist. Ability to use a variety of physical motions in day-to-day activity including standing turning bending stooping lifting and climbing; ability to stand and walk for extended periods of time without experiencing fatigue; ability to accept responsibility and delegate duties; ability to access patient psychosocial needs and coordinate with other staff members; professional in relating to the other medical staff and other departments; ability to keep all information regarding patients and/or families SCHEDULE:Monday - Friday 8:00AM - 5:00PM
