Job Summary

The RN Case Manager is responsible for coordinating patient care across the continuum ensuring appropriate utilization of resources and supporting optimal patient outcomes. This role involves collaboration with interdisciplinary teams to manage care plans discharge planning and patient transitions.

Key Responsibilities

  • Coordinate and manage patient care plans from admission through discharge.
  • Collaborate with physicians nurses and allied health professionals to ensure continuity of care.
  • Facilitate discharge planning including referrals and follow-up services.
  • Monitor patient progress and adjust care plans as needed.
  • Ensure compliance with clinical guidelines and regulatory standards.
  • Document case management activities using systems such as Epic Systems.
  • Communicate effectively with patients families and healthcare teams.

Schedule & Work Environment

  • Day shift (8-hour shifts)
  • Full-time schedule (5 days per week)
  • Weekend requirement: approximately 2 weekends every 6 weeks
  • No on-call requirement
  • Floating may be required based on facility needs

Required Qualifications

  • Active Registered Nurse (RN) license (state-specific)
  • Minimum 2 years of Case Management experience
  • Experience with electronic medical records such as Epic Systems
