Job Summary
We are seeking a compassionate and skilled Registered Nurse (RN) to join our growing Home Health team in Western this role youll provide high-quality patient-centered care in the comfort of patients homesmaking a meaningful impact on their health and independence.
This position offers flexibility strong support through onboarding and a rewarding pay structure.
Key Responsibilities
Deliver skilled nursing care to patients in their homes in accordance with physician orders and care plans
Perform comprehensive patient assessments and ongoing evaluations
Educate patients and caregivers on treatment plans medications and disease management
Collaborate with interdisciplinary teams to ensure optimal patient outcomes
Maintain accurate and timely clinical documentation
Qualifications
Active RN License in the state of Pennsylvania
Associates Degree in Nursing (or higher)
Minimum 1 year of RN experience (Home Health experience preferred)
BLS & CPR Certification (assistance provided if needed)
Valid drivers license reliable transportation and auto insurance
Schedule & Work Environment
Full-time Monday Friday
Flexible scheduling
No long shifts and minimal weekend requirements
Compensation & Benefits
Pay-Per-Visit compensation model
$10000 bonus (paid quarterly)
