We are seeking compassionate and experienced Home Health Registered Nurses (RNs) to provide high-quality patient-centered care in a home setting. This role focuses on delivering skilled nursing services to patients with chronic acute and end-of-life care needs.
Youll play a critical role in improving patient outcomes while allowing individuals to receive care in the comfort of their own homes.
Key Responsibilities- Perform comprehensive patient assessments and develop individualized care plans
- Administer medications treatments and wound care
- Monitor patient progress and adjust care plans as needed
- Educate patients and families on disease management and care plans
- Collaborate with interdisciplinary teams (physicians therapists caregivers)
- Maintain accurate and timely documentation
- Active RN license (NH or compact state)
- Minimum 1 year of RN experience (Home Health or Hospice preferred)
- Strong clinical assessment and critical thinking skills
- Valid drivers license and reliable transportation
- BLS certification required
