Position Overview:
Navitas Healthcare LLC is seeking Licensed Clinical Social Workerfor an exciting Travel or Local job in Atlanta GA.
Requirements
- ActiveGeorgia LCSW license (GA LCSW)
- Minimum2 years of clinical experiencein a hospital or acute care setting
- CurrentAHA BLS certification
- Strong communication assessment and documentation skills
- Ability to work collaboratively within a multidisciplinary healthcare team
Key Responsibilities
- Conduct psychosocial assessments for patients in inpatient and/or emergency settings
- Provide crisis intervention counseling and emotional support to patients and families
- Develop and implement individualized discharge and care coordination plans
- Collaborate with physicians nurses case managers and other healthcare professionals
- Assist with discharge planning including referrals to community resources and support services
- Support patients coping with chronic illness trauma or acute medical conditions
- Maintain accurate and timely clinical documentation in accordance with hospital policies
- Participate in care team meetings and contribute to treatment planning discussions
- Ensure compliance with hospital protocols ethical standards and regulatory requirement
