Position Overview
The Registered Nurse (RN) in a Long-Term Care (LTC) setting is responsible for delivering high-quality patient-centered care while overseeing clinical operations within an assigned unit. This role involves assessing resident health conditions developing individualized care plans administering treatments and ensuring continuity of care across interdisciplinary teams.
Key Responsibilities
Deliver comprehensive nursing care in accordance with care plans clinical standards and facility policies
Conduct ongoing assessments and evaluate resident conditions
Develop implement and revise individualized care plans
Administer medications treatments and clinical interventions safely and accurately
Monitor changes in resident health and initiate appropriate interventions
Collaborate with physicians LPNs CNAs therapists and care teams
Lead patient rounds and ensure clear communication with residents and families
Maintain accurate and timely clinical documentation
Required Qualifications
Education:
Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program
Licensure & Certifications:
Active Registered Nurse (RN) License Required
Basic Life Support (BLS) Certification Required
Advanced Cardiovascular Life Support (ACLS) Preferred
IV Certification Preferred
COVID-19 Vaccination Required
Experience:
02 years of RN experience preferred
Prior experience in Long-Term Care (LTC) or Skilled Nursing Facility (SNF) strongly preferred
