Humber River Healths Nephrology Program is nationally recognized for pioneering the Nocturnal Home Dialysis Programthe first of its kind. The program includes two inpatient medicine units a satellite service at the Reactivation Care Centre and several clinics (Multi-Care Kidney Clinic Nephrology Clinic and Dialysis Clinic) designed to support patients in leading independent and active lifestyles and achieving better clinical outcomes.

Position Objective:

Reporting to the Vice President Clinical Programs the Program Director will lead efforts to drive cultural transformation in our Nephrology and Out-Patient Cardiology portfolio (In-Center Dialysis Home Dialysis Kidney and Transition Care Cardiology Clinic and Respiratory Therapy services) and is responsible for executing the programs strategic priorities while collaborating with physicians and staff to plan and implement key initiatives focused at achieving high-reliability care at Humber River Health. The Program Director will work in partnership with the Ontario Renal Network (ORN) to align and enhance chronic kidney disease services across the Toronto region improving patient care quality and outcomes.

Employment Status:Permanent Full Time

Hours of Work: Day shifts from Monday to Friday (some evening and weekend hours may be required based on operational needs) - subject to change as per Management Rights

Career Experience and Core Competencies:

Masters Degree in Nursing or related discipline registration in good standing with the Regulatory College of area of specialty clinical experience in acute services coupled with at least five years management experience working with a medical dyad partner and experience in business planning resource management program planning development evaluation and managing patient care services is required for this role.
