Job Description:

We are seeking a compassionate and experienced Family Medicine Nurse Practitioner (FNP) to join our dynamic team at Foundation Medical Partners / Southern NH this role you will play a vital role in providing comprehensive care to a diverse patient population in Pepperell MA.

Collegial practice with solid APP support
W2 employed position with Foundation Medical Partners
Not-for-Profit 501(c) Organization
32-36 Patient Hours / plus admin hours per week
Clinical Affiliate of Massachusetts General Hospital
Convenient Southern NH/MA Border Location minutes to the mountains or ocean

Qualifications:

Masters degree in nursing
2 years of Family Medicine experience
Strong clinical skillsincluding assessmentdiagnosisand treatment planning.
Excellent communicationcollaborationand interpersonal skills.
Proficiency with EPIC EMR preferred
Ability to work independently and as part of a team.

Work Shift:

M-F 8am-5pm - position will be in MA
