In this role youll deliver personalized therapy that supports patients in performing daily activities safely and confidently. If youre passionate about functional outcomes and patient-centered care this is an opportunity to do your best work.

What Youll Do

• Provide in-home occupational therapy services to patients and when applicable in assisted living settings
• Evaluate patients physical cognitive and environmental needs
• Develop and implement individualized care plans in collaboration with the healthcare team
• Help patients improve daily living skills mobility and overall independence
• Recommend adaptive equipment and home modifications to enhance safety
• Educate patients and caregivers on treatment plans exercises and long-term wellness strategies
• Document patient progress and adjust care plans as needed

What Were Looking For

• Active Occupational Therapist license in the State of New Hampshire
• Graduate of an accredited Occupational Therapy program
• Minimum of 2 years of clinical experience (home health preferred)
• Current CPR certification
• Strong clinical judgment communication and interpersonal skills
• Ability to work independently and manage a flexible schedule
