This is a high-impact leadership opportunity for an experienced Case Management leader to oversee operations across a large high-acuity acute care hospital. You will take ownership of care coordination discharge planning and resource utilization across a complex fast-paced environment driving outcomes through strong interdisciplinary collaboration and data-driven performance improvement.
Overview
Reporting to the VP of Case Management this Director will lead all aspects of Case Management Social Work and Chaplain services across a 790-bed Level II Trauma facility. This is a visible leadership role responsible for aligning clinical and operational strategy optimizing patient flow and ensuring high-quality compliant care coordination across the continuum.
What You Will Do
- Provide strategic and operational leadership for Case Management Social Work and Chaplaincy services
- Oversee day-to-day operations including staffing workflows and performance management across 110 FTEs
- Lead and develop a leadership team (4 Managers Social Work Supervisor Administrative support)
- Drive care coordination and discharge planning processes to ensure timely compliant and patient-centered outcomes
- Partner closely with executive leadership physicians and post-acute providers to improve throughput and resource utilization
- Analyze operational and clinical data to identify gaps and implement sustainable performance improvement initiatives
- Ensure compliance with regulatory standards policies and best practices
- Collaborate with CFO and system leadership on departmental goals financial performance and strategic initiatives
- Support a high-performing culture focused on accountability communication and continuous improvement
What You Bring
- Bachelors degree required; Masters preferred (Nursing Healthcare Administration or Business)
- RN LCSW or LMSW required
- 3 years of acute care hospital Case Management experience
- 2 years of leadership experience in Case Management
- Proven ability to lead large complex teams and drive measurable outcomes
- Strong analytical skills with the ability to translate data into actionable plans
- Executive presence with excellent communication and relationship-building skills
- Experience collaborating with physicians and post-acute networks
