Responsibilities

• Assist clients with Activities of Daily Living (ADLs) such as bathing dressing grooming toileting feeding and meal preparation.
• Provide companion care and supportive assistance to residents
• Promote safety comfort and emotional well-being
• Observe and report behavioral or health changes to supervisors
• Communicate effectively with staff families and care team
• Maintain a clean respectful and supportive environment
• Follow individualized care plans while maintaining client rights and confidentiality (HIPAA compliant).
• Document care and communication clearly and accurately.
• Support mobility transfers and safe ambulation as needed.
• Engage clients in meaningful activities community outings and social interactions to promote mental and emotional well-being.
• Communicate and collaborate with families case managers social workers and healthcare professionals.
• Document care and communication clearly and accurately.

Skills & Experience

• First Aid & CPR certification (provided if you dont have it)
• Experience in group homes supported living or assisted living highly desirable
• Strong observation skills for monitoring behavioral or health changes
• Ability to lift transfer and assist with mobility (up to 50 lbs)
• Cooking ability for simple and dietary-specific meals
• Home care home health or hospice experience is a plus
• Excellent communication and teamwork skills

Qualifications

• Negative TB test or clear chest X-ray
• LiveScan background check
• Valid drivers license and reliable transportation preferred
