Job Summary

Home Health Field Nurse
A Home Health Field Nurse hold either an LPN or RN nurse who provides healthcare services to patients in their homes. The nurse will assess plan implement and evaluate individualized care plans in collaboration with individuals families and other members of the healthcare team.
Reports to: Executive Director of Home Health
Key Responsibilities (Home Health Nurse RN & LPN)
  • Perform initial evaluation visits for new patients.
  • Regularly reassess and evaluate patient nursing needs.
  • Initiate the plan of care and revise it as needed.
  • Initiate preventive and rehabilitative nursing procedures.
  • Prepare complete and accurate clinical notes.
  • Coordinate services being provided to the patient.
  • Inform physicians and other appropriate medical personnel of changes in the patients condition or needs.
  • Counsel patients and families regarding nursing care and related needs.
  • Participate in in-service training and educational programs.
  • Supervise and teach other nursing personnel as required.
  • Direct and oversee activities of Home Health Aide
  • Accept and carry out oral and written orders from physicians chiropractors podiatrists dentists and optometrists.
  • Assist authorized providers in evaluating the patients functional level.
  • Provide services in accordance with agency policies and home health regulations.
  • Assist physicians or RNs with specialized nursing procedures.
  • Prepare equipment and materials for treatments using proper aseptic technique.
  • Assist patients in learning and practicing self-care techniques.
  • Report changes in patient condition or needs to the physician or authorized provider following consultation with the supervising RN.
Qualifications:
  • Current and valid Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the state of Indiana
  • Graduate of an accredited nursing program.
  • Minimum of one (1) year of nursing experience; home health experience preferred.
  • Current CPR certification.
  • Valid drivers license and reliable transportation.
  • Proficient in electronic charting and documentation.
  • Excellent written and verbal skills
  • Skills & Competencies:
  • Strong clinical and assessment skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently and manage time effectively.
  • Compassionate and patient-centered approach to care.
  • Knowledge of home health regulations (Medicare/Medicaid OASIS documentation preferred).
Working Conditions:
  • Frequent travel to patient homes in assigned territory (25%-50%).
  • May be exposed to varying home environments and patient conditions.
  • Flexible hours based on patient needs and agency scheduling.
While performing the duties of this job the employee is regularly exposed to (i) Individuals residences in which the noise levels are usually moderate and (ii) outside weather conditions.
