A healthcare facility is seeking an experienced Licensed Practical Nurse (LPN) to join its team in a small rural Critical Access Hospital setting. This role involves providing high-quality patient care across Long-Term Care (LTC) and acute/swing bed units. The ideal candidate is adaptable self-motivated and capable of working both independently and collaboratively in a fast-paced environment.
Key Responsibilities
Deliver direct patient care in LTC and acute/swing bed settings
Administer medications and treatments in accordance with physician orders
Monitor assess and document patient conditions accurately
Assist patients with activities of daily living (ADLs) including toileting and routine rounds
Supervise and delegate tasks to Certified Nursing Assistants (CNAs)
Collaborate with Registered Nurses and interdisciplinary care teams
Maintain accurate and timely documentation using electronic medical records (CPSI/Evident)
Ensure adherence to safety protocols policies and quality care standards
Qualifications
Active Licensed Practical Nurse (LPN) license (required)
Minimum of 3 years of nursing experience
Current BLS/BCLS certification (required)
Experience in Long-Term Care (LTC) and acute care settings
Ability to work independently in a rural healthcare environment
Strong communication organizational and teamwork skills
