JOB SUMMARY
Manages the Community Health Assessment (CHA) process and supports Community Health Improvement (CHI) efforts guides data-driven planning and fosters collaborative action.
Leads community health planning by managing the Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) guiding cross-sector partnerships and facilitating data-driven strategies to address priority health needs. Provides leadership and technical support to staff and community partners translates data into actionable goals and clear communications and ensures alignment with organizational priorities while supporting implementation evaluation and continuous improvement.
MINIMUM QUALIFICATIONS
Bachelors degree from an accredited college or university in Public Health Health Education Community Development Project Coordination Public Administration or closely related field plus three (3) years of related experience; OR an equivalent combination of related education and experience.
Preference may be given to applicants with experience in Community Health Assessments (CHA) Community Health Improvement Planning (CHIP) and Mobilizing for Action through Planning and Partnerships (MAPP) or similar community health improvement frameworks.
Due to the nature of this position the successful applicant must pass a required background check through fingerprinting and enrollment in the continuous RAP Back (Record of Arrest and Prosecution Back) program in accordance with current County Human Resources policy requirements.
A valid Drivers License accepted by the State of Utah is required at the time of hire. A valid Drivers License issued by the State of Utah must be obtained within six (6) months of hire to operate a County vehicle.
ESSENTIAL FUNCTIONS
- Manages the Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP).
- Supports the Mobilizing for Action through Planning and Partnerships (MAPP) process; creates maintains and facilitates cross-sector partnerships with the Health Department county and other public agencies community partners and organizations and local coalitions to identify strategic health issues.
- Provides leadership consultation technical assistance and advocacy to Health Department staff community organizations and local coalitions/task forces addressing health priorities.
- Leads internal teams agencies community partners and organizations through structured processes; translates population health assessments and data into actionable CHIP goals measurable objectives and collaborative strategies to address priority health needs.
- Collaborates with agency groups to create tools summaries and visuals that translate data into clear actionable insights for internal and external partners; guides planning performance management and quality improvement while documenting lessons learned and creating templates to support future CHA/CHIP cycles.
- Ensures alignment of CHIP activities with agency strategic goals. Supports community partners and organizations in defining roles responsibilities and evaluation measures for CHIP implementations.
- Facilitates coalition and community partners and organizations meetings liaisons sessions and workshops.
- Coordinates and assists with writing editing and formatting CHA/CHIP reports briefs and communications for diverse audiences.
- Collaborates with Communications team to develop clear public-facing messaging and materials.
- Prepares progress updates and presentations materials for leadership stakeholders and the community.

- Build relationships and foster collaboration across diverse sectors and communities

This is a hybrid position that includes a combination of in-office and remote work.
IMPORTANT INFORMATION REGARDING THIS POSITION
Time-limited Appointed employees will perform work for a specified amount of time dependent on agency funding. They are exempted by law from the rights of a merit employee and work as an at will employee.
WORKING CONDITIONS AND PHYSICAL REQUIREMENTS
May be required to travel to various locations throughout Salt Lake County.
May be required to work evenings/weekends.
