The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and interdisciplinary team collaboration. Responsibilities:
- Provide skilled nursing care to patients.
- Perform patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Provide case management services.
- Deliver hospice care.
- Communicate effectively with patients providers and team members.

Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Bachelors degree in Nursing or related field.
- Clinical Assessment skills.
- Medication Administration skills.
- Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Minimum of 2 years of hospital home health or hospice care experience.
- Drivers license car insurance and automobile for field work.
- Active Registered Nurse license in the state of working.
