- Length: 13 weeks / 40 hrs.
- Available Shift: 08:00 AM to 04:30 PM / 8hrs./day

Pay Rates: -
No housing - Local: $55/hr

With housing (50 miles away from permanent): $42/hr

Housing stipend: $20/hr (taxable) $1480 (tax-free)
(Must be 50miles from facility to taxable address to qualify for housing or housing stipend pay.)

Responsibilities:
- Provide skilled nursing visits to patients in home health hospice palliative care or related community-based settings under physician direction.
- Perform initial and ongoing nursing assessments of patient condition physical status pain symptoms and overall care needs.
- Develop implement and revise individualized plans of care based on patient needs physician orders and hospice goals.
- Monitor and manage pain symptom control comfort measures and patient safety to promote dignity and quality of life.
- Educate patients family members and caregivers on disease process medications treatments comfort care and end-of-life expectations.
- Coordinate care with physicians hospice team members social workers therapists aides and community resources.
- Maintain timely accurate and complete clinical documentation including assessments interventions patient response and changes in condition.
- Participate in interdisciplinary meetings or case conferences to review patient needs and update care plans.
- Support transitions of care and help patients achieve the highest possible level of independence and comfort using Kaiser Permanente and community resources.
- Follow organizational standards for safety quality customer service and patient-centered care.
Qualifications:
- Previous experience in nursing or other medical fields
- RN OR In good standing
- Familiarity with medical software and equipment
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
- IV Certification
- Current - BLS/CPR
- Skills checklist
- Copy or drivers license and SS card
- Must be able to pass background/ urine drug screen
