Description

JOB DESCRIPTION: Accountable for the coordination of care across the continuum for BMT patients and families. Oversees the BMT Plan by collaborating with BMT Physicians and members of the multi-disciplinary team. Manages consistent patient care by assuming the roles of educator resource person researcher and direct and indirect patient care provider. Actively communicates pertinent information to those involved in the care of the patient so that well-informed decisions are made in a timely manner. The position is associated with a significant level of autonomy; therefore the individual must be highly motivated and able to work independently. Demonstrates skill as a leader researcher and educator. Primary Duties and Responsibilities: Coordination and Planning: 1. Organizes and helps facilitate continued care after consultation based on the plan of care for each patient. 2. Obtains insurance authorization and coordinates efforts to establish clinical and financial plans pre-intra-post transplant. 3. Facilitates and coordinates the evaluation of potential transplant candidates. 4. Compiles data for assessment by physicians and advanced practice providers. 5. In conjunction with principal investigator research nurses and advanced practice providers assesses patient eligibility for BMT protocols. Patient/Family Education and Support: 1. Educates patients and families about the transplant process. 2. Functions as the primary contact from the BMT department for the patient and family. Communication and Team Collaboration: 1. Communicates plan to all internal/external team members. 2. Notifies all necessary inpatient/outpatient personnel and insurance case management of all changes in patient schedules for pre-transplant or transplant phase of care and any alternative courses of action that need to be implemented. 3. Notifies team and/or providers of potential barriers to transplant and/or discharge planning issues; i.e. lack of care givers lack of prescription drug coverage etc. 4. Notifies team and/or providers of any changes in the patients clinical condition or pre-transplant schedules. Qualifications: Minimum Required: Education - BSN Experience - Demonstrated experience in Oncology nursing for minimum of 1 year. Licensure - State of Georgia nursing license Certification - 1. BLS Healthcare Provider certification. 2. If completing virtual care activities that may include multi-state practice an active compact/multistate license (eNLC) is required within 60 days of hire. Employees in role prior to 11/09/2025 will have until their next renewal date to achieve compact status. Preferred Qualifications: Education - Masters Degree in nursing. Experience - Prefer case management experience. Prefer Bone marrow transplant experience in an inpatient or outpatient setting for at least 1 year. Knowledge skills and ability requirements (preferred): - Prefer Chemotherapy Biotherapy Certification or obtain within 1 year of employment. - Prefer BMTCTN/OCN or obtain within 2 years of employment.

PHYSICAL REQUIREMENTS (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs 34-66% of the workday (frequently); 01-10 lbs 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking Occasional sitting Close eye work (computers typing reading writing) Physical demands may vary depending on assigned work area and work tasks.

ENVIRONMENTAL FACTORS: Factors affecting environment conditions may vary depending on the assigned work area and tasks. Environmental exposures include but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock Floor Surfaces Hot/Cold Temperatures Indoor/Outdoor conditions Latex Lighting Patient care/handling injuries Radiation Shift work Travel may be required. Use of personal protective equipment including respirators environmental conditions may vary depending on assigned work area and work tasks.
