Licensed Practical Nurse (LPN) Home Health Care

ABA Homecare located in Conyers GA is seeking a compassionate and dependableLicensed Practical Nurse (LPN)to provide high-quality home health care services to patients inRockdale Newton Henry Gwinnett and Fulton counties.

Position Summary

The LPN will provide skilled nursing care in the home setting support patient health goals and work collaboratively with the care team to ensure safe consistent and patient-centered care. This role is ideal for a nurse who values independence flexibility and making a meaningful difference in the community.

Responsibilities

• Provide direct nursing care to patients in their homes.

• Monitor patient condition and report changes to the supervising nurse or care team.

• Provide support to caregivers and patients in the community conducting regular site visits.

• Administer medications and treatments as ordered and within scope of practice.

• Assist with wound care vital signs and routine assessments.

• Document care accurately and in a timely manner.

• Communicate effectively with patients families and interdisciplinary team members.

• Follow all agency policies procedures and applicable state and federal regulations.

• Promote patient safety comfort dignity and quality of care.

Requirements

• Current and active LPN license in the State of Georgia.

• Current CPR & First Aid certification.

• At least 1 year of nursing experience preferred home health experience a plus.

• Strong clinical judgment communication and time management skills.

• Reliable transportation and willingness to travel within assigned counties.

• Ability to work independently in a home care setting as well as with a multidisciplinary team.

• Maintain open line of communication with team members case management and other support staff
