Geriatric Outreach Nurse (RNRPN) (228625)
Job Summary
In accordance with the Vision Purpose and Values and strategic direction of the Vancouver Island Health Authority (Island Health) patient and staff safety is a priority and a responsibility shared by everyone; as such the requirement to continuously improve quality and safety is inherent in all aspects of this position.
Working in the community setting and as part of a multidisciplinary team the Geriatric Outreach Nurse acts as a special resource in psychogeriatric care including concurrent disorders; provides consultation and formal teaching; is responsible for the provision of clinical services to the psychogeriatric client through assessment treatment monitoring and directing care; act as liaison with community agencies and care providers; and assists in community development activities.
QUALIFICATIONS:
Education Training And Experience
Graduation from a recognized nursing program or evidence of an equivalent combination of education training and experience. Three (3) to five (5) years recent related experience including two (2) years relevant clinical experience. Registration with BC College of Nurses and Midwives as a practicing RN registrant or a practicing RPN registrant. Knowledge of current theories technologies and practices in the psychogeriatric care area; of applicable legislation such as the Mental Health Act and Adult Guardianship Act; and of adult education principles. Valid BC Drivers License.
