Job Summary:
Navitas Healthcare LLC is a dedicated Physical Therapist to join a long-term care facility in Concord New Hampshire. The Physical Therapist will evaluate plan and implement individualized rehabilitation programs to help residents improve mobility reduce pain and enhance overall functional independence.
Key Responsibilities:
Evaluate residents and develop individualized treatment plans
Provide therapeutic exercise and rehabilitation interventions
Monitor patient progress and modify treatment plans as needed
Educate patients and caregivers on mobility safety and injury prevention
Maintain accurate and timely clinical documentation
Collaborate with interdisciplinary healthcare teams to ensure optimal outcomes
Ensure compliance with facility policies standards and regulatory guidelines
Qualifications:
Active Physical Therapist license in New Hampshire
Prior experience in long-term care or skilled nursing facility preferred
Strong clinical judgment and patient assessment skills
Ability to work effectively in a collaborative care environment
