Licensed Practical Nurse LPN Weekend Private Duty 1:1 Patient Care

As a Private Duty Licensed Practical Nurse you deliver focused one-on-one care in the comfort of the patients home. You are not managing a hallway of patients. You are providing personalized care where it matters most.

WHAT YOULL DO

Provide direct private duty nursing care in the home
Review Plan of Care and coordination notes before visits
Use clinical judgment to assess and monitor patient conditions
Report significant findings to RN Case Manager or Clinical Team Manager
Document care accurately and submit notes timely
Collaborate with physicians and care teams
Adapt scheduling to meet client needs

WHAT YOU NEED

Graduate of an accredited school of nursing
Active LPN license (state or Nurse Licensure Compact)
Minimum 1 year of clinical nursing experience
Reliable vehicle valid drivers license and current auto insurance
Ability to sit stand bend lift and move intermittently
Ability to lift 50100 lbs as needed
