What Youll Do
- Promote Elara Carings mission philosophy and policies to ensure exceptional patient care
- Provide direct patient care including evaluations treatment planning and outcome assessments
- Design and implement individualized plans of care tailored to each patients needs
- Collaborate with interdisciplinary team members to ensure optimal outcomes
- Direct and support therapy assistants and staff to achieve patient goals
- Maintain accurate and timely documentation including evaluations treatment plans and physician orders
- Serve as a clinical resource and promote physical therapy within the community
- Work in a physically demanding fast-paced home-care environment
- Perform lifting transferring and positioning of patients as required
- May work irregular hours or participate in call rotation if applicable
What Were Looking For
- Graduate of an APTA-accredited Physical Therapy program
- Current unrestricted Physical Therapist license in the state of practice
- Minimum of one (1) year of experience in a healthcare setting
- Willingness and ability to travel up to 50% within the assigned coverage area
- Dependable vehicle valid drivers license and current auto insurance
- Ability to lift push pull and support up to 50 pounds
You will report to the Branch Director Clinical Manager Clinical Supervisor or designee.
