Assists patients and families with complex medical and behavioral health needs to navigate uneventfully efficiently and as effectively as possible through the course of their illness to improve or maintain social emotional functional and physical health. Practices professional social work and promotes patient advocacy in accordance with the NASW Code of Ethics for Social Work.
Essential Functions:
- Functions as a member of an interdisciplinary team and provides care coordination services to patients and their families through screening and identification of patients outreach to primary care physicians specialty physicians and other service providers completion of the Health Risk Assessment and assisting with the development of a family-centered plan of care.
- Facilitates communication and collaboration amongst the healthcare team. Provides consultation education and training to staff regarding psychosocial issues that impact the delivery of optimal health care services. Participates in orientation and continuing education of staff and students as appropriate.
- Develops patients/family self-management skills through education and resource provision.
- Manages transitions between settings caregivers and providers. Monitors and follows-up across the continuum of care.
Education Requirement:
BSW required.
Licensure Requirement:
Must maintain a current Ohio Social Work license.
Certifications:
Active BLS certification required.
Skills:
- Ability to communicate (verbally and written) clearly convey information to team members healthcare providers and families with professionalism and tact.
- Organization time management and prioritization skills.
- Ability to work within a multidiscipline matrix collaborate with other providers and agencies. Ability to relate to diverse age and demographic backgrounds.
- Ability to analyze complex situations identify potential problems and develop effective solutions.
- Computer literate with working knowledge of Microsoft products (i.e. Outlook Work PowerPoint Excel and Teams).
- Working knowledge of Medicaid and other regulatory agency standards desirable.
- Basic knowledge of healthcare terminology.
Experience:
- Experience working in healthcare in a patient facing role preferred.
- Electronic Health Record experience required.
- Working knowledge of Medicaid and other regulatory agency standards required.
Physical Requirements:
OCCASIONALLY: Bend/twist Climb stairs/ladder Communicable Diseases and/or Pathogens Electricity Lifting / Carrying: 0-10 lbs Lifting / Carrying: 11-20 lbs Machinery Patient Equipment Pushing / Pulling: 0-25 lbs Reaching above shoulder Squat/kneel
