What You Will Do

Provide skilled nursing care to patients in their homes
Assess patient health status and identify physical psychosocial and environmental needs
Develop implement and manage individualized plans of care
Conduct initial evaluations and ongoing patient assessments
Coordinate care with physicians and interdisciplinary teams
Monitor patient progress and communicate changes in condition
Maintain accurate and timely clinical documentation

What You Bring

Current Registered Nurse RN license in the state of practice
1 year of clinical nursing experience
Ability and willingness to travel within the branch coverage area
Reliable transportation valid drivers license and auto insurance
Ability to lift push pull or assist patients up to 50 pounds
Associate or Bachelors Degree in Nursing preferred
Home Health or Hospice experience preferred
Experience with HomeCare HomeBase preferred
