Physiotherapist

• Provide ministryfunded communitybased physiotherapy services within clients homes and/or through virtual service delivery
• Complete comprehensive physiotherapy assessments using interviews standardized tools and informal assessment techniques in areas including:
  • Geriatrics
  • Neurology
  • Orthopedics
  • Palliative care
• Develop and implement goaloriented evidencebased treatment plans in collaboration with clients and caregivers
• Assign supervise and monitor physiotherapy assistants when appropriate
• Provide education to clients and caregivers and connect them to appropriate community resources
• Maintain accurate timely and professional documentation including:
  • Assessment findings
  • Care plans
  • Progress and discharge summaries
  • Documentation aligned with referral source requirements and College of Physiotherapists of Ontario (CPO) standards

Qualifications :

• Registered Physiotherapist in good standing with the College of Physiotherapists of Ontario (required)
• Valid G2 or G drivers license with access to a reliable vehicle for community travel
• Communitybased physiotherapy experience is an asset however new graduates are welcome
• Ability to work independently while collaborating within an interprofessional team
• Strong organizational skills including selfscheduling prioritization and meeting reporting deadlines
• Excellent verbal written and reportwriting communication skills
• Proficiency with computer applications including MS Office Suite and proprietary clinical software
