Description of Work
This Social Worker position will be responsible for the provision of social work services within two-three residential units. Caseload serves up to a total of 20-30 adults with IDD many whom may have behavioral challenges and/or medical issues. This position is supervised through CDCs Social Work Department. The Social Worker is an integral part of the treatment team who has the lead responsibility in the facilitation of admissions transitions discharges and guardianship petitions. The social worker completes psychosocial evaluations for new admissions and promotes communication between the guardian/family and the assigned residential teams. This position also works closely with our Olmstead Specialist. Some flexibility for after hour admissions visits appointments court hearings trainings etc. may be necessary.Knowledge Skills and Abilities/Management Preferences
Salary Range: $43 431.00 - $79504.00

This role focuses on supporting adults with intellectual and developmental disabilities (IDD) who also have behavioral and mental health challenges requiring strong knowledge of social work principles and the ability to apply them to complex individual and group cases.
Building effective relationships with residents families coworkers and community partners is essential.
Demands skills in understanding medical terminology psychological diagnoses and appropriate clinical interventions along with strong communication and documentation abilities.
Familiarity with community resources is important and required certifications include CPI training as well as First Aid and CPR.
Management Preferences: The role requires strong communication interpersonal and writing skills along with experience using video teleconferencing and electronic health record systems such as EPIC. Candidates should have training in Person-Centered Thinking and Trauma-Informed Care and experience working with individuals with I/DD who have complex behavioral or medical needs strong case management skills experience with admissions discharge planning guardianship modifications and court documentation. Experience working under ICF-IDD regulations and writing comprehensive biopsychosocial evaluations and case studies for complex referrals is preferred and Electronic Health Care Record System Knowledge (EPIC experience).

Minimum Education and Experience

Masters degree in social work from an appropriately accredited institution; or Bachelors degree in social work from an appropriately accredited institution and one year of directly related experience; or Masters degree in a human services field from an appropriately accredited institution and one year of directly related experience; or Bachelors degree in a human services field from an appropriately accredited institution and two years of directly related experience; or an equivalent combination of education and experience.
**ICF requires that candidates hold a Degree in Social Work. This is an ICF/IDD requirement.**
This position requires a four year or higher degree from an accredited school of social work. Masters degree in social work (MSW); bachelors degree in social work (BSW) and one year of social work experience.
