Your new role
Southend Council are looking to appoint a permanent part-time (18.5hrs per week) Social Care Assessor.
The part-time hours are flexible but you are required to work Wednesdays due to team meetings.
The successful candidate will be required to support individuals who reside in Southend with the aim of offering the appropriate support in the right place at the right time. You will be required to understand an individuals strengths assets and wishes to maximise their potential for independence recognising the importance of assuming that the individual is best placed to know their own outcomes goals and wellbeing.
As well as this you will undertake Care Act 2014 Assessments reviews and support individuals to develop personalised care plans that take account of community assets informal networks universal services and other support and provision which reduces dependency. Support people with specialist needs to have fulfilling lives work with health and social care colleagues and other partners by engaging in innovative practice and a visionary approach to creating a better Southend.
Whilst this role is community based some work will also be undertaken within hospital settings.
What youll need to succeed
To be shortlisted for this position you must have previous experience of working within the community and be familiar with adult social services.
You must have knowledge of applying the Care Act (2014) Mental Capacity Act (2005) and managing adult safeguarding enquires.
You will need to have the ability to apply a strengths-based asset-based approach to assessment care planning and review. Have the ability to develop relationships with internal & external stakeholders including voluntary public and private sector organisations and partners suppliers and contractors.
