What You Will Do

Manage and coordinate your own patient caseload as a Home Health RN
Complete patient assessments and reassessments
Develop implement and update individualized care plans
Perform start of care visits and routine follow up visits
Educate patients and families on treatment plans and health management
Communicate changes in patient condition with physicians and the clinical team
Collaborate with an interdisciplinary team to improve patient outcomes
Utilize electronic documentation systems such as HomeCare HomeBase

What You Need

Current unrestricted Registered Nurse RN license
At least 1 year of experience in a clinical care setting as a nurse
Ability to travel within the branch coverage area
Valid drivers license and reliable transportation
Ability to frequently lift push pull and support up to 50 pounds
Associate or Bachelors degree in Nursing preferred
Previous Home Health RN or Hospice experience preferred
Experience with HomeCare HomeBase preferred
