Schedule: 40 hours/week (8-hour shifts)

Position Summary

Navitas Healthcare LLC is a compassionate Licensed Practical Nurse (LPN) to join a Long-Term Care (LTC) team for the evening shift. The LPN will provide hands-on nursing care collaborate with interdisciplinary healthcare teams and ensure the safety comfort and well-being of residents in a skilled nursing environment.

Key Responsibilities

• Administer medications and treatments as prescribed by physicians
• Provide direct resident care including bathing feeding toileting and mobility assistance
• Monitor document and report resident vital signs health status and responses to care
• Collaborate with RNs CNAs and interdisciplinary staff to ensure resident-centered care
• Assist in developing and implementing individualized care plans
• Maintain infection control standards and ensure a safe care environment
• Serve as a safety companion for residents when required
• Participate in quality improvement initiatives to enhance resident outcomes
• Maintain accurate and timely clinical documentation

Qualifications

• Active LPN license in Wisconsin
• Experience in Long-Term Care or Skilled Nursing preferred
• Strong clinical assessment critical thinking and communication skills
