Social Worker Psychiatry ER (Mobile Crisis) Mount Sinai Morningside Full Time Day (Mondays, Tuesdays, Fridays, Saturdays 1015am 800pm)

Job Title: Social Worker - Psychiatry ER (Mobile Crisis) - Mount Sinai Morningside - Full Time - Day (Mondays Tuesdays Fridays Saturdays: 10:15am -8:00pm)

• Assesses and evaluates patients/families social psychosocial and environmental situations to determine patient needs.
• Demonstrates knowledge of the disease entity and common medical interventions in the service area assigned.

• Conduct home visits in the community.
• Develop options and treatment plans in a timely manner.
• Provide direct service to patients and families selecting treatment modality casework technique compatible with assessment and identified goals.
• Ability to identify select access and make referrals to appropriate community resources.
• Monitor coordinate manage activities for high risk psychosocial patients to ensure implementation of multiple services.
• Advocate for patients and families by interpreting their needs to and eliciting cooperation from other disciplines and community agencies.
• Identify and assist team in managing obstacles to treatment and/or discharge plans.
• Provide psycho-education for patients and families.
• Knowledge of child protection elder abuse domestic violence policies and protocols.
• Participate interdisciplinary rounds case conferences staff meetings.
• Document in the medical record in accordance with standards set by Department of Social Work and/or regulatory agencies.
• May supervise social work interns.
• Coverage (OT) when needed and schedule is subject to change based on the operational needs of the program.

• Masters in Social Work from an accredited school of social work. Knowledge of social work services theories techniques community resources and agencies.
• Related experience in the social service and/or health care field preferably one year in a medical setting which can include related field placement.
• New York State Department of Education: Licensed Masters of Social Work (LMSW).
• Community home visits required.
