What Youll Do
- Conduct meaningful assessments to understand patients social emotional and environmental needs
- Provide immediate crisis intervention in high-stress time-sensitive situations
- Develop and coordinate personalized care and discharge plans
- Support patients and families in coping with illness trauma and hospitalization
- Act as an advocate ensuring patients receive the resources and support they deserve
- Collaborate closely with physicians nurses and interdisciplinary teams to deliver comprehensive care

Additional Job Description
Qualifications:
Masters Degree in Social Work from an accredited program
24 years of experience in healthcare behavioral health or related field
Licensed Social Worker (LMSW) in the state of South Carolina
Basic Life Support (BLS) Certification with American Heart Association required or must obtain upon hire
