Job Title: Physical Therapist
Location: Lackawanna County PA
Job Summary:
The Physical Therapist is responsible for providing high-quality physical therapy services to patients in accordance with a physician-approved plan of care. This role involves evaluating patient needs developing treatment plans and ensuring effective rehabilitation outcomes while maintaining compliance with organizational policies and professional standards.
Key Responsibilities:
- Provide physical therapy services based on a written physicians plan of care.
- Assess and evaluate patients functional therapeutic and rehabilitative status.
- Develop and implement individualized treatment plans.
- Evaluate home environments for safety and suitability.
- Assess muscle strength mobility gait and range of motion (ROM).
- Direct and modify physical therapy treatments as needed.
- Instruct patients and caregivers on therapeutic techniques and use of medical equipment.
- Determine priority needs for therapy and adjust care accordingly.
- Train patients in the use of prosthetic devices when applicable.
- Monitor and report patient progress reactions and changes in condition to the physician.
- Collaborate with healthcare team members and participate in case conferences.
- Identify the need for additional home health services and coordinate referrals.
- Prepare and submit clinical documentation and progress reports.
- Participate in discharge planning and ensure continuity of care.
- Provide consultation and education to patients families and caregivers.
- Conduct or assist with in-service training for healthcare staff.
- Participate in peer review and consultation processes.
- Supervise physical therapy aides and delegate tasks appropriately.
Additional Duties:
- Ensure necessary equipment and resources are arranged for patient care.
- Supervise and evaluate physical therapy aide performance regularly.
- Perform other job-related duties as assigned.
Qualifications:
- Licensed Physical Therapist (as required by applicable regulations).
- Previous experience in home health or community-based care preferred.
- Current CPR certification.
- Strong communication organizational and interpersonal skills.
