Malpractice Must be included in rates.
Orientation $187.50-200 half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly $375-$400 - Per hour not OT
Holiday $562.Must be 1.5x the hourly rate. Per holiday hour no OT
On Call (Daily) $Per 24 hours to include 4 hours gratis
24 Hour Holiday Call Rate - $Per 24 hours to include 4 hours gratis
Call Back $375-$400 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
Holiday Call Back $562.50-600 Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
Average worked Callback Hours: weekday - 8.2h weekend - 9.4h; weekday less than 50% weekday weekends and ER.

Dealbreakers: Low risk high risk and FHR interpretation required. Routine delivery and complicated delivery. Repair of 3rd-4th degree lacerations. C-sections- must be willing to help and supervise if needed. Must be comfortable handling OB inductions from group mostly emergent GYN surgeries.

Credentialing Deadline & Committee Meeting Schedule
The credentialing deadline is three weeks prior to the provider s start date. All required documents must be completed and in hand by this deadline.
Providers who will be on vacation out of town or facing any other obstacles that may delay the credentialing process will not be considered. Please ensure these deadlines are taken into account when discussing start dates only accurate start dates will be accepted.
If your provider has any concerns on their background (e.g. malpractice criminal history etc.) please be aware of the following committee meeting dates when setting their start date:
- Pierce Regional Credentials Committee Meeting (SJMC SAH SCH) Every 3rd Monday (Monthly)
- King Regional Credentials Committee Meeting (SANH SFH) Every 3rd Tuesday (Monthly)
- One Board Executive Committee Meeting (for ALL VMFH) Every 4th Tuesday (Monthly)
Ensuring compliance with these deadlines will help facilitate a smooth credentialing process.

- LI Nursery: late term and term deliveries. Less than 50% callback weekday weekends and ER. 90% low risk pregnancy 10% high risk 20% surgical gyn. No other OB/GYN present during locum coverage. Low risk high risk and FHR interpretation required. Routine delivery and complicated delivery. Repair of 3rd-4th degree lacerations. GYN ER normal scope of practice- DCs Ectopics ER consults etc. High risk patients get transferred to Tacoma. Peds currently done by CNMs/FPOBS. LD staffed with 2-3 RNs. 3 operation rooms on 2nd floor. L&D 3rd floor. C-Sections recovered in LD. Providers include CNMs FPOBs who do C-Sections. Must be able to help and supervise if needed. Anesthesia CRNAs and OR staff have 30 min response time. Currently doing VBACs. Will occasionally receive transfers from local lay midwives. Must be comfortable handling OB inductions from group mostly emergent GYN surgeries. 20-30 deliveries/month high risk 10 or less.
