Job Summary

Job Title: RN - Hospice
Location (On-site Remote or Hybrid): Oconomowoc WI (onsite)
Contract Duration: 13 weeks
Work Hours: 7PM - 7:30AM
Total Weekly Hours: 36

Contract Length: 13 weeks
Start Date: 4/20 or 5/4 (Bi weekly orientations)
Guaranteed Hours Per Week: 36
8 patient visits per shift; approximately spending 1 hour per patient including drive time patient visit and charting- 6 county service area- up to 250 miles per week- average 50 miles per day
Shift Requirements: 12hr Nights; See below:
Specific work pattern would be a rotating 2 week static schedule:
Week 1: Sunday Wednesday Thursday
Week2: Monday Friday Saturday
Weekends or holidays: Every other weekend; Holiday Rotation
Float Requirements: None
On Call/Call Back Requirements: optional
Required Certifications to be Attached upon Submittal: AHA BLS Active WI RN or Compact license
Minimum Experience: Home Hospice exp required. Knowledge of Epic charting system is not required but appreciated.
Strong patient advocate Comfortable with End of Life Hard worker Resilient and Manages Change well Social candidates
