Job Title: Occupational Therapist Home Health
Job Type: Contract (13 Weeks)
Work Schedule: Day Shift 5x8 Hours (8:00 AM 4:30 PM)

Job Summary:
We are seeking an experienced Occupational Therapist (OT) for a home health setting. The ideal candidate will provide one-on-one patient care helping individuals improve daily living skills and overall independence in their home environments.
Key Responsibilities:
- Evaluate patients and develop individualized treatment plans
- Provide in-home occupational therapy services
- Monitor patient progress and adjust treatment plans as needed
- Complete accurate and timely documentation using EMR systems
- Educate patients and caregivers on therapy techniques and home safety
- Manage a daily caseload while maintaining productivity standards
- Travel between patient homes within the assigned service area
Patient Care Expectations:
- 1:1 patient ratio
- Up to 6 patient visits per day
- Productivity expectation: 30 points per week
Qualifications:
- Active Occupational Therapist (OT) license (or eligibility)
- Minimum 1 year of skilled home health experience required
- Experience with OASIS documentation required
- Strong communication and time management skills
- Ability to work independently in a home health environment
Certifications:
- BLS certification required
