Job Title: Physical Therapist Home Health
Job Type: Contract (13 Weeks)
Work Schedule: Day Shift 5x8 Hours (8:00 AM 4:30 PM)
Location: Medford OR
Job Summary:
We are seeking an experienced Physical Therapist (PT) for a home health setting. The ideal candidate will provide one-on-one patient care focusing on improving mobility strength and overall functional independence in patients homes.
Key Responsibilities:
- Evaluate patients and develop individualized treatment plans
- Provide skilled physical therapy services in a home setting
- Monitor patient progress and adjust care plans as needed
- Complete accurate and timely documentation using EMR systems
- Educate patients and caregivers on exercises and safety techniques
- Manage daily caseload efficiently while meeting productivity standards
- Travel between patient homes within the assigned service area
Qualifications:
- Doctor of Physical Therapy (DPT) or equivalent
- Active state PT license (or eligibility)
- Minimum 1 year of skilled home health experience required
- Experience with OASIS documentation required
- Strong communication and organizational skills
- Ability to work independently in a home health environment
Certifications:
- BLS certification required
