Job Summary:

We are seeking a Physical Therapist to provide high-quality care in an outpatient setting. The ideal candidate will evaluate patients develop individualized treatment plans and deliver therapy to improve mobility function and overall patient outcomes.

Key Responsibilities:

Evaluate patients and create personalized treatment plans
Provide therapeutic exercises and rehabilitation programs
Monitor patient progress and adjust care plans as needed
Maintain accurate and timely documentation
Educate patients on home exercise programs and recovery strategies
Collaborate with interdisciplinary teams
Float to other clinics as required

Qualifications:

Doctor of Physical Therapy (DPT) or equivalent
Active state licensure (or eligibility)
Outpatient experience preferred
Strong clinical and communication skills
