Occupational Therapist OT Home Health (PRN)

Empath Home Health a member of Empath Health is seeking a compassionate PRN Home Health OT (Occupational Therapist) to work with our Home Health team to deliver individualized care to patients in their homes in Port Charlotte/North Port and surrounding areas.

Locations: Home health visits in Charlotte county (Port Charlotte North Port Rotonda West Englewood Venice & surrounding areas)

Schedule: PRN; 5-10 home health visits per week

What Youll Do

• Perform comprehensive patient assessments including OASIS assessments at required intervals
• Develop initiate and revise individualized plans of care as patient needs evolve
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes self-care techniques and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless holistic care

What Youll Need

• Active Florida OT license
• Minimum of 1 year experience as a licensed Occupational Therapist (OT).
• Home health care experience strongly preferred.
• Prefers experience using Homecare Homebase (HCHB) EMR
• Strong assessment critical thinking and patient education skills
