Job Summary
Collaborates with Operations Administrator physicians nursing units and NCH leaders to facilitate the patient admission transfer and discharge processes. Monitors and identifies bed availability to support patient throughput and effectively utilizes inpatient beds. Attends bed control meetings in an effort to monitor bed availability daily census and the bed control process. Supports and upholds quality standards of a Magnet designated hospital and Patient Bill of Rights and practices under the RN Bed Placement Supervisor and the Nursing Manager/Director.
Job Specific Duties
- Collaborates with physicians & hospital units to facilitate the patient admission & transfer processes in accordance with NCHS policies/procedures for timely bed placement.
- Completes the admission and patient transfer process to designated nursing unit.
- Participates in bed control meetings provides updates on patient bed placement admissions transfers and discharges.
- Identifies and recommends quality and process improvement initiatives and assists with the development and execution of action plans. F: Daily/ T: ongoing R:
- Compiles reconciles and distributes reports including the bed status report and direct admit list to designated NCH staff.
- Monitors inpatient bed availability facilitating effective utilization of inpatient beds to drive throughput & facilitate the admission process.
- Provides patient information and effectively completes the hand off process at shift change.
- Accesses electronic health record with the support of an RN (Bed Placement Supervisor or transfer center RN) according to hospital policy.
- Promotes a safe error free working environment by consistently performing thorough hand-offs and infection prevention measures.
- Identifies barriers to the admission/transfer process for patients. Suggests solutions to identified barriers and in conjunction with clinical leadership to implement these solutions.
- Coordinates and continuously monitors patient flow supporting patient throughout utilizing appropriate resources from admission transfer or discharge and facilitates the processes in a timely manner including overview of electronic handoff process and acting as a liaison between the ED OR and inpatient units.
