Your Opportunity
We are seeking a Physiotherapist to deliver care across both Community and Residential Aged Care settings.
In this hybrid role youll support older Australians to remain active independent and connected. Whether thats in their own homes or within residential facilities. Youll work as part of a large collaborative multidisciplinary team delivering personalised evidence-based interventions that make a genuine difference.
Using our Plena Care Pathway a reablement-focused model youll help clients achieve meaningful functional outcomes at every stage of their aged care journey.
In the community setting youll work with clients referred through Home Care Packages (HCP) Short Term Restorative Care (STRC) Commonwealth Home Support Programme (CHSP) as well as private clients.
What You Will Be Doing
Youll manage a diverse caseload across both settings including:
- Mobility and functional assessments
- Restorative and reablement therapy
- Falls prevention and risk minimisation
- Pain management and musculoskeletal care
- Pressure care and skin integrity management
- Equipment prescription and home modifications
- Active ageing and group exercise programs
Youll deliver both 1:1 therapy and group-based interventions while collaborating closely with care teams families and other clinicians to provide holistic person-centred care.

What We Are Looking For
- Bachelors degree in Physiotherapy
- Current AHPRA registration
- A passion for reablement and improving quality of life for older people
- Australian working rights
- Willingness to obtain NDIS Worker Screening and a National Police Check
