Licensed Practical Nurse (LPN) Long Term Care (Permanent Placement) Job Summary A large care facility with 212 resident rooms offering long-term care short-term rehabilitation and memory care services is seeking dedicated Licensed Practical Nurses (LPNs) for permanent placement. This role is ideal for local nurses who are passionate about providing compassionate resident-centered care in a supportive environment.
Key Responsibilities - Provide direct nursing care and support under the guidance of Registered Nurses and clinical leadership.
- Assist residents in long-term care short-term rehab and memory care settings with daily medical needs.
- Administer medications and treatments as ordered and in accordance with facility policies.
- Monitor resident conditions and promptly report changes to the supervising nurse or provider.
- Document care observations and interventions accurately and in a timely manner.
- Support interdisciplinary collaboration to ensure continuity and quality of care.
- Maintain infection control standards safety protocols and regulatory compliance.
- Communicate effectively and professionally with residents families and staff.
Requirements - Active LPN license in the state.
- Current CPR and First Aid certifications (required).
- Experience in long-term care skilled nursing or rehabilitation preferred.
- Strong communication observation and clinical skills.
- Ability to work in a team-oriented fast-paced care environment.
- Local candidates only; no travel positions.
