LPN Long Term Care

A large care facility with 212 resident rooms offering long-term care short-term rehabilitation and memory care services is seeking dedicated Licensed Practical Nurses (LPNs) for permanent placement. This role is ideal for local nurses who are passionate about providing compassionate resident-centered care in a supportive environment.

• Provide direct nursing care and support under the guidance of Registered Nurses and clinical leadership.
• Assist residents in long-term care short-term rehab and memory care settings with daily medical needs.
• Administer medications and treatments as ordered and in accordance with facility policies.
• Monitor resident conditions and promptly report changes to the supervising nurse or provider.
• Document care observations and interventions accurately and in a timely manner.
• Support interdisciplinary collaboration to ensure continuity and quality of care.
• Maintain infection control standards safety protocols and regulatory compliance.
• Communicate effectively and professionally with residents families and staff.

• Active LPN license in the state.
• Current CPR and First Aid certifications (required).
• Experience in long-term care skilled nursing or rehabilitation preferred.
• Strong communication observation and clinical skills.
• Ability to work in a team-oriented fast-paced care environment.
• Local candidates only; no travel positions.
