Licensed Practical Nurse (LPN) Rehabilitation (Permanent Placement) Job Summary

A large multi-level care facility with 212 resident rooms offering long-term care short-term rehabilitation and memory care services is seeking skilled Licensed Practical Nurses (LPNs) for permanent placement within the Rehabilitation Unit. This role is ideal for local LPNs who are passionate about supporting patients through recovery and regaining independence.

Key Responsibilities
  • Provide direct nursing care to residents undergoing rehabilitation following illness injury or surgery.
  • Assist with assessments implement care plans and deliver nursing interventions based on resident rehabilitation needs.
  • Administer medications and treatments as ordered by providers and in accordance with facility procedures.
  • Monitor resident progress document changes and report updates to supervising RNs and interdisciplinary team members.
  • Support therapy goals by collaborating with physical occupational and speech therapy staff.
  • Maintain accurate and timely documentation of all care provided.
  • Uphold safety infection control and regulatory standards.
  • Promote a positive encouraging environment that supports rehabilitation outcomes.
  • Communicate professionally with residents families and team members.
Requirements
  • Active LPN license in the state.
  • Current CPR and First Aid certifications (required).
  • Experience in rehabilitation post-acute care or skilled nursing preferred.
  • Strong observation clinical and communication skills.
  • Ability to work effectively in a team-oriented fast-paced rehab setting.
  • Local candidates only (no travel positions).
