Position Summary A skilled and motivated Physical Therapist (PT) is needed for a rapid-response crisis assignment in a long-term care and skilled nursing environment. The PT will play a critical role in evaluating planning and executing therapy interventions to improve patient strength mobility and overall functional ability.
Key Responsibilities - Conduct comprehensive patient assessments to determine functional status and therapy needs.
- Develop and implement individualized treatment plans to promote mobility and rehabilitation.
- Monitor patient progress and adjust plans as needed to ensure optimal outcomes.
- Accurately document patient evaluations treatments and progress in accordance with professional and regulatory standards.
- Collaborate closely with interdisciplinary team members to support coordinated patient care.
- Educate patients and families on therapy goals exercises and expectations.
- Maintain a safe and therapeutic environment.
Qualifications - Active and unrestricted state Physical Therapist license.
- Minimum of 1 year of experience preferably in a SNF or long-term care setting.
- Strong clinical reasoning and communication skills.
- Ability to function independently in a fast-paced rapid-response environment.
- Commitment to excellence in patient care and rehabilitation outcomes.
Additional Details - Guaranteed 36-hour workweek.
