Position Summary
A skilled and motivated Physical Therapist (PT) is needed for a rapid-response crisis assignment in a long-term care and skilled nursing environment. The PT will play a critical role in evaluating planning and executing therapy interventions to improve patient strength mobility and overall functional ability.
Key Responsibilities
Conduct comprehensive patient assessments to determine functional status and therapy needs.
Develop and implement individualized treatment plans to promote mobility and rehabilitation.
Monitor patient progress and adjust plans as needed to ensure optimal outcomes.
Accurately document patient evaluations treatments and progress in accordance with professional and regulatory standards.
Collaborate closely with interdisciplinary team members to support coordinated patient care.
Educate patients and families on therapy goals exercises and expectations.
Maintain a safe and therapeutic environment.
Qualifications
Active and unrestricted state Physical Therapist license.
Minimum of 1 year of experience preferably in a SNF or long-term care setting.
Strong clinical reasoning and communication skills.
Ability to function independently in a fast-paced rapid-response environment.
Commitment to excellence in patient care and rehabilitation outcomes.
