Job Title: Community Living Center Registered Nurse (RN)

Location: 10701 East Blvd Cleveland OH 44106

Shift Schedule:

• 8 10 or 12-hour shifts
• Rotating Days Nights Weekends Holidays
• 24/7 coverage environment

Shifts per month: 20 (FTE-based)

License: Ohio RN or Compact

Certifications: BLS

Experience needed: Minimum 2 years long-term care / SNF / rehab experience

Special Skills:

• Geriatric and chronic disease care
• Medication administration and monitoring
• Care planning for long-term residents
• Patient/family communication

Responsibilities:

• Deliver continuous nursing care to long-term residents
• Monitor chronic conditions and administer medications
• Support care plans focused on comfort stability and quality of life
• Prevent complications such as infections pressure injuries and falls
• Maintain detailed EMR documentation and coordinate with interdisciplinary teams

Misc:

• Lower acuity but high patient load and continuity care focus
